Family Caregiver Model


Bridges firmly believes individuals and their families must be able to choose where their services are delivered and have more control over who delivers those services. We help families who seek to provide services directly to a family member with a disability; either in their own home or another appropriate setting in their community.

Bridges ensures each supported person is provided greater service flexibility, more choice and broader living options. Bridges supports family members interested in this model by providing a unique and heightened level training monitoring and support.  Although under this model, most direct caregivers are family members, often another person who lives in the home, who may not be family, is eligible to provide daily care just as the natural family would.
